Nothing on it is an offer or an invitation to the general public to purchase or self-administer these products.

6. The FORMA AI assistant

FORMA AI answers from a knowledge base about FORMA products. It can be wrong, it does not give medical advice, and it is not a substitute for the instructions for use or for a conversation with our team. Anything it says that matters commercially or clinically should be confirmed with us in writing.

7. Reporting a problem with a product

If you are a practitioner and a patient experiences an adverse event associated with a FORMA product, report it to us at info@theforma.it and to the competent authority in your country. Please include the batch number from the label.

8. Governing law

This notice and any dispute arising from the use of this site are governed by Italian law, with exclusive jurisdiction in the courts of Milan, save where mandatory consumer-protection rules provide otherwise.
